Form: collect answers without leaving the game

An input screen at the right moment in the path: sign-up, feedback, a photo taken in the field. The answers land in your stats.

How it works

The Form step shows the fields you choose: short or long text, number, email, phone, date, dropdown, checkbox, multiple choice, file upload. Players fill them in and submit, like any web form.

Built into the game, the form saves you from sending people to another tool, and you get their answers right next to their results. You add it in the editor, at the point in the path where the question makes sense.

For the player

  1. 1The player sees the fields, with an asterisk on the required ones.
  2. 2They type, check boxes, pick from a list or drop in a file, such as a photo.
  3. 3They submit. A missing or invalid field, like a bad email or phone number, is flagged right below it.

What you set

  • The fields: label, type, required or not, default value.
  • Settings for each type: options for a dropdown or multiple choice, min and max for a number, a latest allowed date.
  • On an email field, an automatic email to the address entered: subject, a message that can reuse other answers ($name, $team…) and an attachment.
  • Each field's name, which identifies it in your stats and in the CSV export.

Ideas for using it

  • Workshop sign-up

Collect name, team and workshop choice at the end of a kickoff game.

  • Training feedback

Ask for a rating and a comment right after the last challenge.

  • Proof from the floor

Have players upload a photo of a tidy workstation or a checked piece of equipment during a safety walk.

  • Send a cheat sheet

Automatically email a summary sheet as an attachment to the address the player entered.

Frequently asked questions

How do I add a form to a game?

In the Ludiz editor, add a Form step where you want it, then create your fields one by one. Players fill it in without leaving the game.

Where do I find the answers?

In the game's stats, player by player. The CSV export adds one column per form field.

Can the form send an email?

Yes. On an email field, turn on sending: when the player submits, the address entered gets the message you wrote, with an attachment if you need one.
